Participation Agreement, Health Declaration and Waiver
You will have consented to the below ahead of your first booking. A reminder of the details below:
1. Assumption of Inherent Risk
I acknowledge that Pilates is a form of physical exercise and carries inherent risks, including but not limited to muscle strain, joint sprain, falls, aggravation of pre-existing conditions and other physical injury. I understand that Classes and 1:1 Sessions using equipment (such as the Reformer, Cadillac or Spine Corrector) carry additional mechanical hazards, including pinch points, spring tension, carriage movement and strap slippage. I voluntarily choose to participate in Classes and/or 1:1 Sessions and accept the risks that are inherent in Pilates when it is properly taught and supervised.
2. Health Declaration
I confirm that, to the best of my knowledge, I am physically able to participate in Pilates Classes and 1:1 Sessions on the Mat and using the Pilates equipment. I have disclosed to the Teacher all relevant health conditions and injuries, including but not limited to heart conditions, high blood pressure, joint problems, osteoporosis and pregnancy, and I will tell my Teacher promptly if my health changes. Where I have a medical condition, an injury or recent surgery, or am pregnant, I confirm I have sought the advice of my doctor or other healthcare provider before commencing or returning to Pilates. I understand that Pilates Teachers are not medical professionals and cannot give medical advice.
3. Medical Emergency
In a medical emergency, I consent to Studio staff calling the emergency services on my behalf and sharing with them any health information I have disclosed.
4. Liability
Nothing in this agreement excludes or limits the liability of The Pilates Collective Surrey Limited for death or personal injury caused by its negligence, for fraud, or for any other liability that cannot be excluded or limited by law, and nothing in it affects my statutory rights as a consumer. Subject to that, I agree that The Pilates Collective Surrey Limited, its owners, teachers and staff are not liable for: (a) injury or loss resulting from the inherent risks described in section 1; (b) injury or loss caused by my failure to follow instructions, to disclose relevant health information, or to comply with Studio policies; or (c) loss of or damage to my personal property, except where caused by the Studio's negligence. This section applies to Classes and 1:1 Sessions at The Studio, Chobham Community Centre and Sunningdale Heath Golf Club, and to online Classes.
5. Instruction Compliance
I agree to follow all instructions provided by the Teacher and understand that I am responsible for my own body and safety during a Class or 1:1 Session. I will only adjust equipment with my Teacher's guidance and permission, and I will listen to the orientation and safety precautions. I will notify my Teacher of any discomfort or pain during a Class or 1:1 Session.
6. Cancellation and Refund Policy
Classes or Sessions cancelled more than 24 hours in advance will be recredited to the relevant Membership or Package; cancellations made less than 24 hours before the start time receive no credit. There are no refunds for individual Classes, Memberships or Packages; Memberships and Packages are non-transferable and Packages are not extendable. Clients must cancel direct via their account. Full details are in the Terms & Conditions on the website. This policy does not affect my statutory cancellation and refund rights.
7. Studio Policies
I understand that for all Classes there is a strict cut-off for late arrivals of 5 minutes past the start time, after which no one will be admitted; this is for my safety and the enjoyment of other participants. For all Reformer Classes and 1:1 Sessions in The Studio I will wear grippy socks, for safety and hygiene, and clothing without back zips so as not to damage the equipment. I understand that if I do not follow these policies I will not be able to participate in that Class or 1:1 Session and no credit will be given.
8. Media Consent
I understand that from time to time I may be asked to consent to a Class or Session being photographed or filmed for promotional purposes (namely website and social media content). I will always be asked at the time, and I have the right to say no.
